Carefully read the notice. There will be an enclosed form 15106 included with your notice that the IRS would like you to complete. Mail the completed form and the stub from your notice to the IRS within 10 days.
If you would like to be removed from the IRS third-party list, you will need to either call or send a letter to the IRS.

The IRS requests that you respond within 10 days of the date of the notice.
